
A Guide to Addressing Resident Goals, Preferences and Priorities
Over the past two decades, many nursing homes have worked to improve the way they gather residents’ goals, preferences, and priorities (GPPs). The Moving Forward Coalition heard from nursing home staff and residents and learned the GPP process, including who is involved, can vary greatly across nursing homes.
The purpose of this Guide is to support nursing home staff in addressing each resident’s GPPs by offering resources and recommendations to implement a standardized process.
With this Guide, nursing home staff should be able to:
- Describe how the model articulated in the Guide builds on or goes beyond person-centered care experiences
- Learn ways in which identifying, documenting, and delivering resident care preferences promotes person-centeredness in nursing homes
- Create opportunities for improving person-centered care that align with resident GPPs for care, services, and environments
Note: The GPP Guide specifically focuses on processes addressing a resident’s GPPs only and is not intended to address the entire care planning process.
